This plug-in is exactly what I was needing and it couldn't have been simpler to use. Just open your image file (.bmp, .jpg, .png, etc.) in paint.net, resize it to the appropriate dimensions, remove the background (if it needs to be transparent), and "Save As" .ico file type...piece of cake.
